Medicare Wellness Nurse (RN)

Position Summary: The annual wellness nurse collaborates with primary care providers and other members of the healthcare team to complete Annual Wellness Visits (AWV). The nurse must understand health prevention and promotion topics required in the AWV and be able to identify health maintenance and education needs of patients. The nurse will then formulate individualized plans of care in concert with the primary care providers.


Essential Duties and Competencies:

· Exceptional communication skills

· Review of AWV schedule to ensure patients are due for an AWV

· Develop knowledge of required AWV health maintenance and health prevention topics

· Understanding of clinical guideline recommendations involved in an AWV (i. e. vaccinations)

· Ability to work independently and use appropriate clinical judgement in interactions with patients

· Obtains needed health information and records pertaining to an AWV as needed to close care gaps/promote continuity of care

· Competency in performing various clinical tasks as needed such as vaccine administration

· Confidentiality – Comply with HIPAA regulations to maintain patient and employer confidentiality.

· Flexibility in schedule to meet patient and primary care provider needs.

· Excellent organizational and time management skills

· Knowledge of appropriate documentation in the electronic health record

· Documents in patient health record in a timely manner

· Ability to assist primary care providers with AWV billing.

Required Licensure/Certifications

· Kentucky Registered Nurse Licensure

· BLS Certification


Work Environment/Physical Demands

· Possible exposure to bodily fluids, communicable diseases, toxic substances and conditions common to a medical office environment

· Interaction with a diverse patient population and various members of the health care team

· Frequent walking and standing

· Pushing and pulling possible for patients with impaired mobility requiring assistance



Medical Lab Technologist (MLT)
  • Provides medical laboratory diagnostic and therapeutic information, products, and services by establishing specimen preparation procedures; developing and implementing analytical procedures; evaluating laboratory information; consulting with physicians; reporting results according to protocols mandated by state and federal guidelines.
  • Maintains quality results by participating in the quality assurance program; consulting with physicians; performing proficiency surveys; reviewing quality control and quality assurance programs; making adjustments in policy and procedures; generating reports; maintaining records.
  • Complies with state and professional continuing education licensure requirements by providing in-service programs, monitoring outcomes.
  • Resolves problems by consulting with the lab director, lab manager, lab personnel, physicians, nurses, and other healthcare professionals.
  • Maintains state and national credentials according to state and federal guidelines.
  • Contributes to team effort by accomplishing related results as needed.
